The survey will take about 2 minutes to complete and at the end of the survey you will be automatically entered to win 1 of 10 $200 gift cards given away monthly. To start the survey you will need to locate the 12 digit SALESCHECK # on your receipt. You will also need the ASSOC#, the RINGING ASSOC# and the date of visit. All of this information can be found on your Sears Hardware receipt.
