The Off Broadway Survey will take about 5 minutes to complete. To take the survey you will need:
- date of your store visit
- which Off Broadway store did you visit (state and store number)
- the total amount spent (Rounded to the nearest dollar, i.e. “50”. Please do not include the $ sign)
