Take the LensCrafters Customer Feedback survey and you will be automatically entered into a sweepstakes for a great prize! LensCrafters.com/Survey is available in English, Spanish or French Canadian and should only take a few minutes to complete. Please note that you do not need to make a purchase to enter the sweepstakes. The lucky sweepstakes winner will take home a LensCrafters Gift Certificate for a complete pair of prescription eyeglasses or prescription sunglasses valued at Approximate Retail Value (“ARV”) $500 (the “Prize”). The drawing is sponsored by Luxottica Retail North America Inc and you must be 18 years of age or older in order to win. The winner will be notified within 14 days of the drawing and will receive their prize in 6 weeks.
