- Place of purchase (online, US or Canada)
- State or Province or purchase
- The transaction number from your Crazy 8 customer receipt
The transaction number should be 11 digits long and can be found towards the bottom of your receipt. The survey is available in English or Spanish and it does not appear that it can be taken over the phone. At the completion of the Crazy8Cares survey you will be entered into a drawing for a $500 gift card.
