It’s faster and easier to complete your Breast Implant Follow-up Study survey ONLINE with your registration code and date of birth. If any issues arise please dial toll-free at 1-866-619-BIFS. Once you have provided your registration code and date of birth you will be able to participate in the BIFS survey. The BIFS Power of One Survey is operated by Allergan.
