Whats the deal?
If you take the AMF Bowling survey you will receive a coupon for $5 off your next visit. The survey must be completed with in 3 days of your visit and you have 30 days to use the coupon or it will expire. The $5.00 off coupon is valid for bowling only and requires a $5 minimum purchase. The survey coupon cannot be used with any other promotions. To begin the survey you will need the 5-digit number found at the top of your survey invitation from AMF Bowl. If you do not have Internet access please dial 800-294-5919 to take the survey by phone. You will still get the coupon even if you take the survey over the phone. The AMF Survey is managed by Mindshare Technologies (a survey software company).
