The Banner Dependent Eligibility Verification site allows a customer to verify dependents in the Banner Health medical, dental and vision plans. All users who have one or more dependents enrolled in a Banner Health plan will be required to verify their eligibility. For questions in regards to the verification process or any other issues please dial verification process (877) 894-2696 between the hours of 9AM-5PM Central time Monday-Friday. A user can visit the Banner Health Internet Verification Assistance Center iVAC site to obtain the status of documents submitted, update a mailing address or upload document images.
Banner Health eligible dependents are defined as follows:
- The dependent resides in the US
- A person of the opposite sex who is legally married to an employee
- Common Law Marriage as defined by the employees state
- Children, stepchildren, adopted children or those of whom the Banner Health employee has legal guardianship over up to age 26
Please note that children are defined as a natural child, a stepchild, legally adopted child or a child legally placed for adoption. If the employee is claiming a legally adopted child the employee will have to provide State or county documents to provide the adoption along with the issued birth certificate showing employee’s name or signed court order.
